- รูปถ่ายขนาด 1 นิ้ว (1 inch photo, 2 pictures) 2 รูป
- สำเนาบัตรประจำตัวประชาชน (1Copy of ID card) 1 ใบ
- หนังสือรับรองการจดทะเบียนจัดตั้งบริษัทฯ 1 ชุด (ครบชุด) (Certificate of incorporation)

ค่าลงทะเบียนเช้าเป็นสมาชิกและค่าบำรุงสมาคม (Registration Fee and Membership Fee)
1.ประเภท VIP นิติบุคคล (VIP Member)
- ค่าลงทะเบียน (Registration Fee) 500 บาท (500 Bath) ชำระครั้งเดียว (One-time Payment)
- ค่าบำรุงสมาชิกรายปี (Annual Fee) 39,000 บาท / ปี (39,000 Baht per Year)
- รวมเป็นเงิน (Total) 39,500 บาท (Baht)
2.ประเภทสามัญนิติบุคคล (Ordinary Member)
- ค่าลงทะเบียน (Registration Fee) 500 บาท (500 Bath) ชำระครั้งเดียว (One-time Payment)
- ค่าบำรุงสมาชิกรายปี (Annual Fee) 2,000 บาท / ปี (2,000 Baht per Year)
- รวมเป็นเงิน (Total) 2,500 บาท (Baht)
3.ประเภทวิสามัญ บุคคลธรรมดา (Extraordinary Member)
- ค่าลงทะเบียน (Registration Fee) 500 บาท (500 Bath) (ชำระครั้งเดียว) (One-time Payment)
- ค่าบำรุงสมาชิกรายปี (Annual Fee) 1,000 บาท / ปี (1,000 Baht per Year)
- รวมเป็นเงิน (Total) 1,500 บาท (Baht)
